If you are a new patient you will be offered a telephone appointment with one of the partners to welcome you to the practice. If you have any of the following conditions below you may be offered an appointment for a bloods and a review with a nurse as soon as possible and not wait as usual for your birthday month:
Asthma
Atrial Fibrillation
Chronic Obstructive Pulmonary Disease (COPD)
Dementia
Diabetes
Epilepsy
Heart Failure
Mental Health
Osteoarthritis
Chronic Pain
Parkinson’s Disease
Rheumatoid Arthritis
Stroke or Transient Ischaemic Attack
Underactive Thyroid (Hypothyroidism)
Health checks for Patient with no long term conditions
The NHS Health Check is a health check-up for adults in England aged 40 to 74. It’s designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia.
If you’re in the 40 to 74 age group without a pre-existing condition, you should receive a letter from your GP surgery or local council inviting you for a free NHS Health Check every 5 years. You can also email or call us to book a Health Check.
