New service available to all patients-
Would you like a review of the medications you are on by a pharmacist or pharmacy technician with a special interest in your medical problem?
Have you recently been discharged from hospital and are confused about what medications have been changed and why?
We now have two lovely pharmacist colleagues Phil and Julie working with us on Fridays and they have a special interest in the following areas:
Allergies and Asthma
COPD
Recent discharge from Hospital and medicine safety
Patients who are Complex and on multiple medications
Safety of medications in Frail and elderly patients
Medications in Housebound patients
Please ask to book with either Phil or Julie to discuss your medications in a face to face or telephone appt or in a home visit
