Waiting list for patients who “able to travel” or are “happy to travel at short notice” for the Covid 19 vaccine

IMPORTANT MESSAGE

ARE YOU ABLE TO TRAVEL TO OTHER LOCATIONS OR WOULD YOU BE WILLING TO TRAVEL AT SHORT NOTICE TO CHURCH STRETTON OR SHREWSBURY/TELFORD IN ORDER TO GET YOUR COVID VACCINE EARLIER WITHIN YOUR PRIORITY GROUP ?

We know that many of you are very keen to have the Covid-19 vaccine as soon as possible.

Initially at least supplies to our practice may be limited and we may not be able to invite all the patients we would like to quickly. It may be that more vaccine is available in other locations within South-west Shropshire such as Church Stretton than at the practice in Ludlow initially.

It may be that sometimes we get told at very short notice that there is vaccine available in Church Stretton or even Shrewsbury or Telford. We don’t want to throw away any of this precious vaccine.

Therefore we need to know who is able to travel to onother location or is available at short notice to travel in order to access the vaccine. With this in mind we have made this short form for you to fill in in order to register your interest and availablity to travel at short notice to other locations for the vaccine.

It is absolutely important that we have a mobile number that we can text the invitation to. If you don’t have a mobile number we will accept any mobile number of a friend, family member or neighbour who will receive an anonymous message from us with a tel number to ring to book the appt. No personal data will be shared other than the tel no to call or a website link to follow.
